when in medschool, do you have a gpa or anything of the sort. does it really matter if you are getting an A or a B for when you want to match for your residency?

yes most schools have GPA and class ranks, a few have gone to P/F .
From AMCAS:AMCAS GPAs are almost always different from those calculated by the schools you have attended. Therefore, AMCAS does not in any way attempt to compare our calculations with those appearing on your official transcripts.

we didn't have a GPA, we had the F/P/HP/H
we did however have class rank and our class rank was reported on our transcript.
whether or not your GPA or class rank makes a difference in residency (if at all) mostly depends on what specialty you're doing to apply to. main things besides grades they are going to look at for residency are your USMLE scores, Dean's letter and/or recommendation letters.
