I took bio 1 w/ lab and chem 1 w/ lab 15 years ago and got F's in both (suffered from depression that semester and couldn't focus on studies). After that I changed my major from bio to psych, got a BA in Psych (3.8 GPA in psych courses) and got an MBA degree (3.6 GPA). I'm considering medicine as a second career and have to take the science prereqs. Should I be concerned that I failed those courses and got a .57 GPA for that semester? Assuming I do very well on my prereqs this time around do you think addmissions committees will look past the previous failures?

They were 15 years ago and wont really matter at a DO school. they are much more forgiving and willing to look at what you have done lately, not 15 years ago. Also becuase they get a lot more applicants doing medicine as a 2nd career choice, id assume they see many people like yourself. If you get A's for the most part in all your pre reqs you will be fine for a DO school.
